Hospital Discharge helps older people settle back in at home after a hospital stay.

This free service lasts for up to 28 days and is designed to help reduce the risk of older people being readmitted to hospital.

Our experienced and professionally trained workers are fully insured and DBS Police checked. They’ll make sure that everything is in place for the best possible recovery. 

We can move furniture to make space for medical equipment, fit keysafes to enable carers to gain safe access and provide information and advice on benefits and entitlements.

We also check in with the discharged person in the weeks after leaving hospital to find out how they’re getting on and help resolve any issues that come up in that time.
